Nomination Form  

Create the Nomination Form by completing the fields above. Next check the box below that indicates that you have read and accept the TTCA Futurity Rules. Finally click the "E-mail Form" button below. This will send the completed form to IvyglennTTs @ aol.com. To complete your registration, write a $25.00 check payable to TTCA, and mail it to the address below (no SASE is necessary). You will receive an email from me confirming receipt of your Nomination Form. After your check for $25.00 is received, you will be emailed your Official Litter Number, which indicates the successful completion of your Futurity registration.
